Several online survey services are available to faculty, instructors and researchers for academic purposes. Online surveys allow the respondents to enter their responses by filling out a form on the web. Once this form has been submitted, an application gathers and analyzes the responses automatically. It is easy to create surveys, view the results, and to do other analyses.

Checkbox Online Survey Tool
Checkbox, by Prezza Technologies, Inc., was adopted in Auguest 2007 to replace the WebSurveyor service. Checkbox allows users to create, deploy, and analyze surveys in any browser.
Blackboard Survey Tool
The Blackboard course management application can be used to create and deploy surveys from within a course Blackboard site. If you are already managing a Blackboard site, you can create a survey through the course Control Panel. To request a Blackboard account, fill out the IT Request Form.
Secure “Once-only” Survey Tool (OnSet)
The OnSet tool was created at Cornell to create secure surveys. You can control access to a set of respondents through their netID and Kerberos authentication. You can also restrict users to filling out the survey only once. Response data is available via downloading of a delimited text file. Survey questions are text-only. Images are not supported. The Cornell University Committee on Human Subjects (UCHS) exists as a safeguard to promote ethical and responsible treatment of human subjects/participants in research. As the institution's official Institutional Review Board (IRB), UCHS must review and approve all research projects that use human subjects -- regardless of the source of funding -- before the investigator may commence the study. Please refer to the UCHS web site for more information, and to fill out the appropriate application, if necessary.
You don't need to have final approval from the UCHS before beginning the technical work on your survey. However, if approval is required, it must be in place before you can ask participants to fill out the survey.
Things you should consider include: the level of security required to fill out the survey; the method for creating the survey and for viewing the results; and the audience for your results.
