Checkbox

Checkbox online survey software, by Checkbox Survey Solutions, Inc. was adopted by Cornell as a supported survey tool to replace the WebSurveyor service. This commercial web application enables you to create and deliver online surveys, collect responses, and provide summary statistics and graphs. It supports many different question types, and it can create multi-page and branched surveys. Most users find Checkbox to be easy to learn. While users can export the data and import into a full-featured statistical or spreadsheet program, many users will find the built-in analysis tools sufficient. While it is possible to restrict the respondents to a certain list of logins and passwords, Checkbox does not provide truly secure authentication.

Who can use Checkbox?

Checkbox is available to current Cornell faculty, staff, and students for academic purposes only. Currently enrolled Cornell students may use Checkbox  for course assignments, but their instructor must submit the request for the student account. Student accounts are active for one semester.  Respondants can be anyone with access to a web browser.

Request a Checkbox Account

All current Websurveyor users were automatically given Checkbox accounts.  You should have received an email in early August '07.
To all others, you can request a Checkbox account by filling out the IT Request Form.

Support

Alert: In setting Report security permissions for the survey, DO NOT check the option: “Allow access to summary and detailed results”. Enabling this option will allow anyone to view your survey data. If you are a survey administrator, you will be able to view the results without enabling this option.

We are working with the vendor to resolve any confusion in regard to this setting to avoid compromising the privacy of your survey data.

Guidelines for use

  • User accounts are available to faculty and staff for instructional and research purposes.
  • Accounts are subject to renewal every academic year
  • You can have 10 active surveys per account
  • Surveys have a 4 MB size limit
  • You must adhere to the Human Subjects Information Regarding Surveys

Location and Hours

  • Computing and Communications Center Room 124 Garden Ave.
  • Open Monday-Friday 9:00AM - 5:00PM during the academic year
  • Closed on university holidays

Maintenance Schedule

Any regular system maintenance and server restarts for any of our services will be scheduled weekdays between 5:00 am and 7:00 am. All users should be aware that a system may become unavailable during this maintenance time.
